Do link URLs stay in the output?
If 'Keep links' is on, yes: each <a> tag becomes 'anchor text (URL)'. Turn it off and only the visible link text remains, with the address removed entirely.
What happens to codes like & and <?
With 'Decode HTML entities' enabled, they convert into the real characters & and <. When it's off, the entity codes are left in the text exactly as they appear.
